RE: FORMAL DISPUTE AND DEMAND FOR DELETION — 15 U.S.C. § 1681i
Disputed furnisher(s): [COMPANY BEING DISPUTED]
Consumer: [Your Full Name]

To Whom It May Concern:

This is my initial written dispute. I am the consumer identified above. I dispute the following item(s) appearing on the file you maintain in my name. Each item is disputed as inaccurate, incomplete, or unverifiable, and each is disputed on its own independent grounds so that no item may be treated as duplicative, frivolous, or irrelevant under 15 U.S.C. § 1681i(a)(3).

DISPUTED ITEMS

1. UNAUTHORIZED INQUIRY
   Furnisher / Creditor: [creditor]
   Account / Reference: [account number]
Identified on my file: September 18, 2026
Reason disputed: I never signed an application, contract or written authorization with this party, so no permissible purpose existed under 15 U.S.C. § 1681b(a)(2) when my file was accessed.
Claim: I did not authorize this inquiry and no permissible purpose existed.
Demand: DELETE this item in its entirety.

LEGAL BASIS

• 15 U.S.C. § 1681b(a)(2) — a consumer report may only be furnished with my written instructions or a permissible purpose.
• 15 U.S.C. § 1681b(f) — it is unlawful to obtain a consumer report without a permissible purpose.
• 15 U.S.C. § 1681n / § 1681o — willful and negligent noncompliance carry statutory and actual damages.

• 15 U.S.C. § 1681i(a)(1)(A) — you must conduct a reasonable reinvestigation within 30 days and delete any item that cannot be verified.
• 15 U.S.C. § 1681i(a)(5)(A) — information that is inaccurate, incomplete, or cannot be verified must be promptly deleted or modified.
• 15 U.S.C. § 1681i(a)(2)(A) — you must forward all relevant information I provide to the furnisher; a coded e-OSCAR two-digit summary is not a reasonable investigation (see Cushman v. Trans Union, 115 F.3d 220).

WHAT I DEMAND, IN ONE STEP

1. Delete each item listed above rather than "verifying" it through an automated response.
2. If any item is retained, provide, per 15 U.S.C. § 1681i(a)(6)(B)(iii) and § 1681i(a)(7): the method of verification, the business name, full address and telephone number of every furnisher contacted, and the name of the individual who verified the data.
3. Provide a free updated copy of my file after the reinvestigation, per 15 U.S.C. § 1681i(a)(6)(B)(ii).
4. Do not treat this dispute as frivolous; each item is separately identified with the specific basis of dispute as required by § 1681i(a)(3)(B).

I have not authorized any third party to file this dispute; it comes directly from me. Please respond in writing to the address above only. Failure to complete the reinvestigation within 30 days requires deletion as a matter of law.

Sincerely,
